The Care Coordination Manager at Kennestone Regional Medical Cente ris responsible for providing leadership and managing all processes/services within Care Coordination; assumes responsibility for implementing standards of practice, evidenced based medical guidelines, departmental policies and procedures, and maintaining and improving the quality of patient care delivered; assures staff and program compliance with established standards, practices, policies/procedures and regulatory requirements; responsible for overall standards of patient care, financial performance, human resource management and staff engagement, patient/customer satisfaction, appropriate reporting and for coordinating program functions with the operations of other hospital departments to assure quality services at a reasonable cost for patients; responsible for directing continuum of care practices that address the biological, developmental, psychosocial and clinical needs of the patient and/or family. Creates an environment which enables the system to fulfill its mission and goals, fosters communication and collaborative practice with physicians and other departments.
Required Minimum Education : Graduate of an accredited school of Nursing. B.S.N. is preferred. Will consider years of experience in case management in lieu of Bachelor’s or Master’s degree.
Required Minimum Experience: A minimum of 3 years’ experience in the management of a multidisciplinary staff in the field of case management or quality and patient safety with at least 3-5 years of experience in a hospital environment is required.
Required Minimum Certification : Current RN license in the State of Georgia, or LMSW. BLS is required.
Required Minimum Skills: Strong interpersonal skills to facilitate team dynamics are necessary. Time management skills, leadership skills, prioritization skills and change management skills are necessary to focus on goal achievement for the system.
